First he pretended to already be in a coma and then had the wear-with-all to drop a bottle to signal the staff?

He wasn't pretending. He was really in a coma from the 'double' insulin injection that the villain gave him. He didn't have 'wherewithal to drop' it -- his arm accidentally swept across the table and he knocked the glass/bottle of milk over as he was going into the coma, which is what ended up saving him, because it caught the attention of the nurse who got the doctor to come in who gave him a sugar injection to counteract the insulin, which brought him out of the coma.
I just saw this movie today for the first time.
