Ilsa refers to Sam, played by 56-year-old Dooley Wilson, as "the boy who's playing the piano."

What she means by "boy" is an african american/black man.

The term is an archaic one from a different era. Plus, Ilsa is European and that term was strictly an american one. Makes me wonder where she picked it up!

Likely so (in the scriptwriters' mind), although "boy" was also used to refer to any grown man who was considered a servant, in the U.S. and U.K. In one of Myrna Loy's movies from the '30s she addresses a white man older than herself who's a waiter or something like that as "Oh, boy, can you...."
