What was it that the doctor was being suffocated in? Was this grain mill or some sort of metallic powder?

Probably wheat flour.

I think it was a grain mill of some kind. The gears and wheels turning made me think that.

It's a Flour mill. You can be suffocated like that. People have died falling into storage tanks of grain.
