who was he? If they explained how he got it I missed it. Did they say? It looked like the Hitchhiker's from Chain Saw. Also how important was he to the plot? I drifted in and out.

He was the doctor who acted as a sort of Greek chorus.

The character had been a war surgeon in WWI and had a grenade explode in his face. He thereafter was in a perpetual state of shell shock, it appears.
