What was with the doctor?

I haven't seen this movie for a while but if I recall correctly, Alonzo sort of calls in a favor with the doctor, knowing he did something before that I guess he could be ruined for, and if he gives Alonzo the surgery, he won't tell. If that's correct, can somebody tell me if they ever figured out WHAT the secret was?

At first I thought maybe the doctor was somehow responsible for Alonzo's double-thumb. But I guess we're just supposed to infer that he did something pretty terrible. Creepy stuff!

What's the Spanish for drunken bum?

Alonzo is blackmailing the Dr. The Dr. has a secret from his past that he doesn't want Alonzo to expose. It is not mentioned in the movie what the secret is.

At least one reel is missing from the existing prints of the film (which explains why this supposedly 63 minute long feature is now only 50 minutes). I think it's very likely that Alonzo's relationship with the doctor was established early in the film (perhaps similar to how the doctor/patient relationship is shown in Chaney's THE PENALTY from seven years earlier) and those scenes are among the missing footage.

You're probably right; reportedly the missing reel also shows Alonzo murdering Cojo and the doctor so that the secret about his operation (and his past) is safe.
