I enjoyed this film but don’t understand the title exactly.

The so-called "shock" occurs at the end of the movie, where it appears everything is about to go wrong, but then there's an earthquake. The shock is the earthquake, in that it arrived out of nowhere and actually saved the day. It was shocking in that it appeared there was no way Chaney and the girl would be saved.
