She looks great, but I can't tell if it's a medical issue or is she just enjoying herself a little bit?

If she looks great, who cares?

I agree with you here.

Obesity is unattractive, because obesity indicates poor health, but curviness and a healthy appetite is sexy because it indicates that a person isn't starving themselves to look good.

LOL, why would you suspect she has a medical problem? She gets a little heavy at times, but not so much that I'd suspect something has gone haywire in her body.

She's nearly 40 years old and has given birth to two kids. Also, apparently she gained weight intentionally for Black Mirror:
