Heroína de alquitrán negro. Tom Sizemore started doing Black Tar as early as 1992/93 and keep going for years. He was a functional Black Tar addict.
Heat, Natural Born Killers, Striking Distance, Strange Days all shot while he was high on Tar.
He was forced to get clean for Saving Private Ryan. This lasted into Enemy of the State where he had gained considerably weigh as he substituted food for Tar.
He relapsed before filing Bringing Out the Dead and the rest is history.
He has been doing Tar for 30 years.

I don't doubt it. He looked especially messed up in Red (2008).

Unconfirmed rumors claim Tom Sizemore was smoking freebase and shooting heroin on set. This created an "unsafe environment" The cast complained to the director and the director to the President Eamonn Bowles over at Magnolia Pictures. Since much of the film was shot the decision was to replace the director and move the production from California to Maryland. Tom Sizemore was kept on board but his part was trimmed.
This might explain his tiered appearance as he was high was fuck -or coming down.
