Every picture I see her in, she never seems to smile. If there is any attempt at a smile, it just seems insincere.

That's because she knows too much (Sean Penn to Charlie Rose, July 2nd, 2004 :)). Her presence is very comforting, and she looks comfortable within her sui generis poise. I have not seen Fay Grim yet.

To be honest I haven't seen her in a film in years and I didn't recognize her, When I saw her in Louis I was blown away by her performance, and was curious to see who this "unknown" was. When I saw her name in the credits I thought "ah, that explains it".
