Her eyes look uneven in Maleficent?

I know that this beauty has aged, but even with make up, she didn't look good/healthy?

My missus and I thought that her eyes looked uneven in height, whereas I'd never seen this before? Looking at her in Dangerous Liaisons (1988) she looked fine.

has she had surgery at some point?

Very few people have a perfectly symmetrical face.

I don't know her cosmetic procedure history but I don't see obvious signs that her eyes have changed position:

To me she looks great for a 61 year old.

She does at award shows etc, but here in the film she looks odd somehow...

No one is symmetrical. She was asymmetrical in scarface too

Everyone has some asymmetry, some are more notable than others.

I imagine the aging process has maybe made it a tad bit more noticeable.
