How was the mirror-in-mirror scene in Inception achieved?

Probably wasn't a mirror there to begin with, I know when she touched the glass and it shattered that was digital effect.
Might have been a case of just taking the shot and flipping it onto a tracked surface or running a separate camera to emulate the angle of the mirror.
Maybe they just shot it and painted out the camera crew, could be one, all or none of these.
In retrospect I probably should have paid more attention to the angles, that where the giveaway would be.

If a second camera is involved, how is the matching-up of body movement between the two footages achieved (from front and behind)? Would they have the actors redo the scene until the illusion was perfect?

If it was that easy, why haven't I seen this kind of technique before in a movie?

You would use two cameras at the same time and use the timecode to align the movement.

I think they probably just painted the camera out in post. Yes, as someone said, it'd be a pain... but hey, digital effects are a pain.

This stuff you guys are talking about with second cameras and the infinite reflections being digitally put in... I don't think so. Knowing the way Nolan likes to do things, he probably wanted to get the infinite reflections in-camera. Painting out the camera wouldn't be much harder than removing wire-work and such, and that's stuff gets done all the time (particularly for a certain part of this film).
